You will have received a copy of the Alex Lilleystone Privacy Policy which sets out

how your data will be processed and stored. Although the records that I make are private and confidential between us there are a few rare circumstances when I am legally bound to share the information that I hold. These circumstances are summarised by the following list of conditions.

1. Where I am seriously concerned about your safety and where disclosure may save you from serious harm;

2. Where I have reason to believe that another person is at imminent risk of serious harm and where disclosure may protect that person.

3. Where I have reason to believe that a child is suffering or is likely to suffer significant harm and that disclosure may protect that child.

4. Where a court orders me to share the information that I hold about you.

If I have to share your information, I will share the minimum amount necessary for the purpose (as far as is required by the law) and I will do so preferably with your consent and if this is not possible, with your prior knowledge, so long as this is consistent with the reason for the disclosure. UK laws regarding privacy and data protection affect all Psychologists, Counsellors and Psychotherapists equally. However, I am not a law enforcement agency. For example, if you disclose misdemeanours, petty crime, or fantasy, all that you share is absolutely private and confidential between us, given the provisions 1-4 above. Lastly, all psychologists share their clients’ information with a supervisor who is similarly bound by laws regarding privacy and data protection.

I must stress that the conditions listed 1 to 4 above, are exceptionally rare. I will not share your information with anybody unless these conditions are met. If you are concerned about what these conditions mean and you would like to discuss kinds of circumstances that might be included within one of the above conditions you are welcome to raise the topic. I will provide as much information as I can to help you to understand the meaning of the conditions.

I endeavour to support the self-determination of all my clients, while being aware of the limitations placed upon each individual to determine his or her life outcome. You are free to undertake this counselling work or to choose not to do so at any time. I value honesty, accuracy and fairness within all my work. I will seek to be honest and fair in all that I convey to you, including:

• My professional qualifications and experience;

• My conclusions and opinions (including any potential limitations of them).

This psychological counselling service will end:

• When you decide that it is no longer benefitting you;

• In exceptional circumstances, if I believe that you are not deriving benefit from the service. If appropriate, I will refer you for an alternative service. I will not end the service without attempting to discuss my reasons with you.

• You may prefer that we define the number of sessions that we intend to complete together at the commencement to the work. E.g. six or ten sessions.

• I request that you attend on time. If you are going to be late, please let me know as soon as possible. If for some reason, such as illness, I am unable to attend the session I will tell you with as much notice as I can provide. We will not be able to start the sessions earlier than the time that we are due to start.

• If you wish to book a therapy service, I require a £75 returnable deposit, which secures your next appointment.

• If you cancel an appointment less than 24 hours prior to the appointed time, or if you do not arrive for your appointment you will be required to pay a non- attendance fee of £75 to me and to replenish the £75 cancellation deposit. I regret this, especially if your non-attendance was unavoidable (e.g. sudden illness or traffic problems). However, non-attendance of appointments is a considerable expense for me.

• You agree to pay the fees as stated in the table below.

• I don’t have facilities to take a card payment but cash is acceptable.

• Each session will last 55 minutes, starting at the appointed time. Please do not arrive early or you may interrupt another session.

• If you arrive late for a session, I will remain available for the planned duration of the session, however, it is not possible to extend sessions beyond the set ending time.
